A. This is a really fascinating question that has undoubtedly changed the geopolitical face of the planet. Because Europe was more densely populated than other continents during the early-middle ages, when diseases landed in Europe, they tended to "hang around" and infect the same places over and over again. Without the proper antibiotics and supportive care to help people recover, most people infected with, say, bubonic plague simply died - which means they couldn't have more children. Natural selection meant that people who got the plague, but whose biology was somehow resistant to the plague, were able to survive and go on to have children - these children would also be more likely to be resistant to the plague.
